Targeted Case Management (TCM) helps individuals access medical, social, educational, vocational, and housing resources to ensure basic needs are met. By providing personalized services and support plans, Guild’s TCM team can help you improve your physical and mental health.
Our Targeted Case Management team partners with you to help create a plan based on your individual needs and preferences. Our goal is that you meet your goals. We know that receiving customized services can help you get what you want out of life when you have schizophrenia, bipolar disorder, major depression, or another psychiatric illness.

- adult mental health Targeted Case Management
MHR’s Targeted Case Management (TCM) teams provide person-centered support to adults who have been diagnosed with a Serious and Persistent Mental Illness. MHR provides personalized service for people in the community who may have co-occurring medical, social and substance abuse challenges.
- children’s mental health Targeted Case Management
MHR’s Children’s Targeted Case Management teams provide support to children who have been diagnosed with Severe Emotional Disturbance and their families. MHR provides person-centered, personalized services for youth who may have co-occurring medical, social, and substance use challenges.

Targeted Case Management is a service that helps individuals access medical, social, educational, and other supports. A case manager works with clients to assess needs, develop service plans, and connect them to appropriate resources.
To qualify for Guild’s TCM services, you must:
- Be 18 years or older.
- Live in the Twin Cities metro area.
- Have a severe and persistent mental illness diagnosis.
For individuals with Medical Assistance (Medicaid), TCM is usually covered. For others, sliding scale fees or other funding sources may be available.
