What Are HCBS?
Home and Community-Based Services (HCBS) provide tailored support to individuals with disabilities, aging adults, or those managing health conditions. These services are essential for individuals who want to live at home or in their community rather than transitioning to institutional care.
Through our HCBS program, Roadway helps individuals maintain their independence while ensuring they receive the care and resources they need to lead fulfilling lives.
Our Services
Roadway offers a variety of Home and Community-Based Services, including:
- Individualized Home Supports (IHS): Assisting individuals in their homes with daily living tasks, health management, and personal goals.
- Independent Living Skills (ILS): Helping individuals develop and strengthen essential skills, such as budgeting, cooking, self-care, and effective communication, to enhance their independence.
- Homemaking:
Providing help with household tasks, including cleaning, meal preparation, laundry, and maintaining a safe living environment.
- Respite Care: Offering temporary relief to family caregivers by providing short-term care for individuals, ensuring their needs are met while caregivers take a break or attend to other responsibilities.
These services are tailored to meet the specific needs of each individual, ensuring comprehensive and compassionate care.
Who We Serve
Roadway’s HCBS program is available to individuals across Minnesota who:
- Have a disability, chronic illness, or condition that impacts daily living.
- Wish to live independently in their homes or communities.

- Are eligible for Medicaid or related assistance programs.
